Cryo Studio Sarasota Inc. (“we,” “our,” or “us”) is a Florida corporation that operates two consumer brands: Cryo Studio, an in-studio body contouring and aesthetic practice (cryostudiosarasota.com); and The Anti Aging & Wellness Institute (“AAWI”), a telehealth weight management and wellness program (theantiagingandwellnessinstitute.com). This Privacy Policy explains how we collect, use, share, and protect information when you visit either website (each, a “Site”) or enroll in any of our programs or services (collectively, the “Services”).
By using a Site or our Services, you acknowledge that you have read and understood this Privacy Policy.
We collect the following categories of information:
Information you provide directly. When you contact us, request information, complete an intake, or enroll in our Services, we may collect your name, date of birth, contact information (email address, phone number, mailing address), demographic information, health and medical history relevant to the Services you are requesting, current medications and allergies, lifestyle and eligibility information, and payment information.
Information collected automatically. When you visit the Site, we and our service providers may automatically collect certain technical information, including your IP address, browser type, device identifiers, operating system, pages viewed, referring website, and timestamps. We use this information to operate, secure, and improve the Site.
Information from third parties. We may receive information about you from our clinical partner, your prescribing provider, the dispensing pharmacy, payment processors, and other service providers that support the delivery of the Services.
We use your information to:
Provide, administer, and improve the Services, including coordinating telehealth consultations, prescriptions, and medication fulfillment through our clinical partner and contracted pharmacy network;
Verify your eligibility for the Services, including verification of your state of residence;
Process payments and refunds;
Communicate with you about your account, treatment, appointments, refills, and customer support inquiries;
Send you marketing and educational communications about our Services, where permitted by law and consistent with your communication preferences;
Maintain the security, integrity, and proper functioning of the Site and Services;
Comply with applicable laws, regulations, legal process, and our legal obligations, including those under federal and state healthcare and telehealth laws.
We do not sell your personal information. We do not use or disclose your health information for marketing purposes without your written authorization where such authorization is required by law.
We share your information only as necessary to operate the Services and as described below:
Clinical partner and providers. We share information with our clinical platform partner and the licensed healthcare providers who evaluate your case, in order to deliver the medical components of the Services. These providers maintain their own clinical records and are subject to their own professional and legal obligations regarding patient information.
Pharmacy. We share prescription and shipping information with the licensed pharmacy responsible for dispensing and shipping your medication.
Payment processors. Payment information is processed by PCI-DSS compliant third-party payment processors.
Service providers. We share information with vendors who provide technical, analytics, communication, and operational support to us, under contractual obligations to protect your information and use it only for the purposes we authorize.
Legal and safety disclosures. We may disclose information when required by law, subpoena, court order, or regulatory request, or when we believe in good faith that disclosure is necessary to protect our rights, your safety, the safety of others, or to comply with a legal investigation.
Business transfers. In the event of a merger, acquisition, financing, or sale of all or part of our business, your information may be transferred to the successor entity, subject to the protections of this Privacy Policy.
We do not sell your personal information, and we do not share your health information for the targeted advertising purposes of third parties.
Information that relates to your past, present, or future physical or mental health, the provision of healthcare to you, or payment for healthcare is “Protected Health Information” under the federal Health Insurance Portability and Accountability Act (“HIPAA”). Your Protected Health Information is handled in accordance with HIPAA and applicable state health-information laws, and is held by our clinical partner as the platform of record for telehealth encounters.
You will receive a Notice of Privacy Practices from the applicable clinical provider at the time of your intake or first clinical encounter, where applicable to the Services you receive. The Notice of Privacy Practices describes your rights under HIPAA in detail and how your Protected Health Information is used and disclosed for treatment, payment, and healthcare operations.
The Site uses cookies and similar tracking technologies (such as web beacons and pixel tags) on general informational pages to operate the Site, remember your preferences, measure site performance, and support marketing analytics. These technologies do not collect health information, and we do not deploy advertising or analytics trackers on intake forms, eligibility screens, or any other pages where you submit health information.
You can control cookies through your browser settings. Disabling cookies may affect the functionality of certain parts of the Site.
Depending on your state of residence, you may have rights regarding your personal information under applicable state privacy laws, including the California Consumer Privacy Act, the Virginia Consumer Data Protection Act, the Colorado Privacy Act, the Connecticut Data Privacy Act, the Florida Digital Bill of Rights, and similar state laws. Patient health information rights under HIPAA — including the right to access, amend, and receive an accounting of disclosures of your Protected Health Information — are addressed by the applicable clinical provider’s Notice of Privacy Practices.
We retain your information for as long as necessary to provide the Services, comply with our legal and regulatory obligations, resolve disputes, and enforce our agreements. Health-related and clinical records are retained in accordance with applicable federal and state healthcare record-retention laws. Marketing and lead information is retained for up to three years from your last interaction with us, after which it is deleted unless retention is required by law.
We maintain reasonable administrative, technical, and physical safeguards designed to protect your information against unauthorized access, disclosure, alteration, and destruction. These include access controls, encryption in transit, vendor due diligence, and contractual safeguards with service providers that handle your information. No method of transmission over the Internet or electronic storage is fully secure, and we cannot guarantee absolute security.
The Services are not directed to, and we do not knowingly collect information from, individuals under the age of 18. If you believe a minor has provided us with personal information, please contact us and we will take appropriate steps to delete the information.
